The Independent Policing Oversight Authority (IPOA) has initiated investigations into the death of Simon Warui, who passed away while in police custody at the Central Police Station, Mvita Sub-county, Mombasa County, on Wednesday, September 17, 2025.
According to IPOA, a post-mortem report revealed that Mr. Warui died from cardiorespiratory collapse resulting from a fracture or dislocation of the neck consistent with a fall from a height.
The Authority stated that this medical finding would guide ongoing inquiries into the circumstances that led to the tragedy.
Evidence Collection and Investigations
The oversight body confirmed that its investigation team has already begun interviewing witnesses and reviewing police procedures related to the incident.
IPOA emphasized that it will take into account all available evidence before making conclusions, promising a transparent process aimed at establishing accountability.
“The safety and rights of individuals placed in police custody are paramount, and IPOA will work diligently to ensure justice is served,” the Authority stated.

IPOA Chairperson Ahmed Issack Hassan underscored the seriousness of the case, assuring Kenyans that the Authority is committed to handling the matter with integrity.
He extended condolences to the family of the late Simon Warui, pledging that IPOA will pursue justice on their behalf.
